Upcoming 2026 Transition for MiHealthLink Dual Eligible Program

MI Health Link is a complete integrated health care program for Michigan residents that meet program requirements and that:

  • Are aged 21 or over. 
  • Live in the Michigan counties of Barry, Berrien, Branch, Calhoun, Cass, Kalamazoo, Macomb, St. Joseph, Van Buren, Wayne or any county in the Upper Peninsula.
  • Are enrolled in both Medicare and Medicaid.
  • Are not residing in a state-operated veteran’s home.
  • Are not currently enrolled in hospice.

On January 1, 2026, the Mi Health Link program will be transitioned to MI Coordinated Health, a” highly integrated dual-eligible special needs plan” (HIDE SNP).  The new plan has a Medicare Advantage framework and goes live in 2026 for beneficiaries in Barry, Berrien, Branch, Calhoun, Cass, Kalamazoo, Macomb, St. Joseph, Van Buren, Wayne or any county in the Upper Peninsula, and will expand statewide in 2027. MDHHS has awarded contracts to nine plans for the new program and these contracts will run for seven years, followed by three, one-year optional extensions. MDHHS has conducted listening sessions so that beneficiary feedback is considered in advance of the transition at the beginning of 2026.
