This training provides the foundation of the BCBSM/BCN PDCM billing codes and Priority Health care management codes. It includes practical application demonstrations and activities to promote confidence in using the codes.
Self-Management Support: Patient Engagement. Nov. 19, Dec. 11
During this live, all-day training course, the learner will gain the knowledge and skills to engage and empower patients through enhanced communication. The attendee will have the opportunity to engage in role-playing, a “simulation interview” with a trained, standardized patient, and receive immediate, individualized feedback on their applied skills.
Serious Illness Training. November 10
This is an all-day live learning experience designed to empower healthcare providers with the expertise to guide compassionate, meaningful, and effective care for their patients.
This evidence-based course equips you with essential tools and strategies to master the delicate art of having serious illness conversations. You’ll learn to navigate complex discussions with ease, help patients understand their diagnosis and prognosis, and honor their unique wishes and values when facing challenging medical decisions.

During this live, all-day training course, the learner will gain the knowledge and skills to collaborate in a multidisciplinary setting to ensure coordinated, patient-centered care.
Patient Experience to Enhance Patient Engagement- Oct. 7, Nov. 4, Dec. 2
During this live training course, the participants will learn practical tools and strategies to enhance care coordination, improve communication, and ensure patient safety and satisfaction. This training is intended to support continuous quality improvement efforts in the patient’s experience of care.

Practice Transformation Institute (PTI)
Community Health Worker Program Opportunities
About the program:
Practice Transformation Institute (PTI) is an approved entity by the Michigan Department of Health and Human Services (MDHHS) to provide training for individuals seeing CHW certification. We are the only CHW program in Michigan that is accredited by the International Accreditors for Continuing Education and Training (IACET) and able to award CEUs for each activity. PTI’s program includes a robust curriculum of 103 hours of virtual classroom learning that meets the requirement of the National C3 Council. Topics include communication, motivational interviewing, action planning, social determinants of health, community resources, chronic conditions, healthy lifestyles, health promotion and disease prevention, and more.

Community Health Worker Sustainability Program
This new two-part program will:
- Identify services for which a CHW can bill
- Explain the requirements for billing the primary payors in Michigan
- Discuss how to utilize HCPCS codes to report services provided
- Describe how ICD-10-CM Z-codes are used to report social determinants of health
- Discuss potential functions and workflows in the organizational setting
- Identify documentation requirements and more
The program is designed for CHWs and other members of the health care, social services, or community-based organization team who regularly interact with CHWs or who wish to gain a greater understanding of the value of CHWs within the team.

Patient Engagement Foundations
About the course:
The goal of this course is for all Care Team Members to learn engagement tools/skills in order to have productive conversations with patients about their health including basic motivational interviewing skills.
The Care Team Member will build upon this foundation, to utilize patient engagement skills in different situations such as Medication Assisted Treatment, (MAT) and Palliative Care.
It is strongly recommended to take Introduction to Team Based Care before taking this course.

Community Health Worker Training Program
The curriculum is designed to train front-line health workers to provide culturally responsive services in Michigan communities. The 166 training hours includes 25 hours for independent self-study. Independent self-study ensures that the participants are given sufficient time to work on homework, quizzes, projects, and other assignments. Independent self-study hours may be completed at the participant’s home, work location, or other location.
As part of the 166 training hours, 40 internship hours are required, consisting of field experiences in local agencies where the focus is for the student to apply and integrate theory into practice. CHWs who are currently employed will complete the internship at their place of work.
Tuition varies by certification training programs and is set by the training organizations, not by MiCHWA

HP offers Introduction to Team Based Care and Patient Engagement Foundations trainings throughout the calendar year. All 2026 training dates are available for registration through the registration link below. Both trainings are 8:30 a.m. to 5:00 p.m.
